Can I Masturbate After Kidney Stone Surgery?

Patients ask this far more often than they say it out loud. It usually comes at the end of the consultation, or not at all, and then gets typed into a search bar at 11 PM instead.

The short answer is yes. There is no medical prohibition, nothing you can damage, and no risk of disturbing the surgery. The only real question is when it becomes comfortable rather than whether it is safe, and that depends on which procedure you had and whether a stent is in place.

The Short Answer by Procedure

After RIRS or URSL, where a scope passed through the urethra, most patients are comfortable within a few days once burning on urination has settled. With a stent in place, it is often uncomfortable, and many prefer to wait until it is removed.

After PCNL or mini PCNL, where there is a small wound in the flank, the limitation is physical exertion and abdominal straining rather than anything urinary. Most patients wait until the wound is comfortable, usually around a week.

After ESWL, where nothing enters the body at all, there is no urethral irritation. The usual advice is simply to wait until any pain and blood in the urine have settled, typically a few days.

In all cases, wait until any catheter has been removed, urinary bleeding has largely settled, and you are off strong painkillers.

Why There Is No Medical Restriction

Nothing about sexual activity, solo or otherwise, interferes with a kidney, a ureter, or a healing urinary tract.

A DJ stent cannot be dislodged or pushed out of position. It is held in place by a coil at each end, one in the kidney and one in the bladder.

Kidney stone surgery does not involve the reproductive organs, the prostate, or the nerves responsible for erection. Those nerves run alongside the prostate, which is why prostate surgery carries that risk and stone surgery does not.

What You May Actually Notice

These are common, expected, and settle.

Discomfort or burning on ejaculation. The most frequent complaint after RIRS or URSL. The urethra has had instruments passed through it and the lining is irritated. It usually settles within a few days to a week.

A dragging ache in the flank. With a stent in place, contraction of the bladder allows a small amount of urine to travel back up toward the kidney, briefly raising pressure. This can produce an ache on the operated side.

More blood in the urine afterwards. Physical activity of any kind increases bleeding while a stent is in place, because the stent moves against the bladder lining. It usually settles within a day.

Occasionally a small amount of blood in the semen. Uncommon, related to urethral instrumentation, and it typically resolves within a week or two. Alarming to see, rarely significant. If it persists beyond a couple of weeks, get it checked.

Urinary urgency afterwards. Again a stent effect, from the lower coil resting against the most sensitive part of the bladder.

None of these indicate damage. They indicate a urinary tract that has recently been instrumented.

After PCNL Specifically

This is the procedure where the considerations are genuinely different, and it is worth separating out.

PCNL involves a small tract through the flank into the kidney. There is a wound, which means the limitation is mechanical rather than urinary. Abdominal straining and vigorous physical exertion in the first week can cause discomfort at the wound site and, rarely, bleeding.

The practical guidance is the same as for other physical activity after PCNL. Wait until the wound is comfortable, avoid anything strenuous in the first week, and build back gradually rather than resuming at full intensity.

Does It Affect Healing or Stone Passage?

No. It does not slow healing, it does not affect the position of a stent, and it does not interfere with the clearance of residual fragments.

Fertility is not affected by any kidney stone procedure. Neither is erectile function, for the reasons above. When to Wait

Hold off and speak to your urologist if you have:

• Heavy bleeding or clots in the urine, as opposed to a pink tinge.
• Pain that is worsening rather than settling as the days pass.
• A catheter still in place.
• Difficulty passing urine.
• A fresh or uncomfortable PCNL wound.
• Specific instructions from your surgeon, such as after a ureteric injury or bilateral surgery. Those take precedence over general advice.

Practical Points

Discomfort is information, not danger. If something hurts, stop. You will not cause harm by continuing, but there is no reason to push through pain during recovery.

Stay well hydrated. It helps with everything in this period.

Empty your bladder afterwards. Simple, and it reduces irritation.

Expect a temporary increase in blood in the urine if a stent is in place, and do not panic when it happens.

And ask your urologist if something concerns you. This is a routine post-operative question and you will not be the first person that week to raise it. A surgeon who cannot discuss ordinary recovery in full is not giving you complete aftercare.

Once Recovery Is Complete
