Kidney stones range from grains of sand you would never notice to masses that fill an entire kidney and weigh more than a kilogram. That is an enormous range, and where your stone falls on it decides almost everything about what happens next.
The frustrating part for patients is that the number on the scan report rarely comes with an explanation. A report saying 14mm means something very different from one saying 4mm, but nobody explains why, and the internet gives contradictory answers.
This article covers the full range, what each size actually means, how stones get large in the first place, and why size alone does not decide your treatment.
The Full Range of Kidney Stone Sizes
At the small end, stones can be barely visible, essentially fine gravel or sand. Many people pass these without ever knowing they had a stone, sometimes noticing only a brief episode of burning or a moment of discomfort.
Most stones that cause symptoms sit between 2mm and 10mm. This is the range that produces the classic picture of severe flank pain travelling toward the groin, nausea, and blood in the urine.
Above 10mm, stones increasingly stay where they are. They stop being a passage problem and become a removal problem.
At the extreme end, stones can exceed 5cm and fill the entire collecting system of the kidney. Case reports in the medical literature describe stones weighing well over a kilogram, though these are rare and usually reflect decades of neglected disease. What is not rare in Indian practice is the 3cm to 5cm staghorn stone discovered in a patient who never had severe pain.
How Stone Size Is Measured on Your Report
This confuses almost everyone, so it is worth explaining.
Your scan report usually gives two measurements, something like 8.4 x 6.2 mm. That is the stone viewed in two dimensions, and the larger number is the one used for treatment decisions. A stone described as 8mm is a stone whose largest measurement is 8mm.
Two other numbers on the report matter and are frequently ignored.
Hounsfield units estimate stone density on a CT scan. A dense stone takes far longer to fragment with a laser than a soft one of identical size, and very dense stones respond poorly to shock wave treatment.
Location. A stone in the ureter behaves completely differently from one sitting in the lower calyx of the kidney, even at identical size.

Why Diameter Is Misleading
Here is the point most size articles miss entirely.
Stone volume scales with the cube of the diameter. A 20mm stone is not twice the material of a 10mm stone. It is roughly eight times.
This is why treatment recommendations change so sharply across what looks like a small range of numbers, and why a patient with three 10mm stones faces a different situation from a patient with one 10mm stone despite similar sounding reports.
It also explains why the jump from 10mm to 15mm matters more than the jump from 3mm to 8mm, even though the second gap looks larger on paper.
What Each Size Range Actually Means
Under 4mm. The large majority pass on their own, usually within a couple of weeks. Management is fluids, pain control, and patience.
4mm to 6mm. Passage becomes less certain. At 5mm the odds are close to even, and at 6mm they tilt against you. Many patients in this range are offered a defined trial period with an alpha blocker before deciding on treatment. 7mm to 10mm. Spontaneous passage is uncommon but not impossible, and it depends heavily on location. Stones sitting low in the ureter, close to the bladder, still pass sometimes. 10mm to 20mm. These require treatment. The question is which procedure, not whether. 20mm to 40mm. Larger stone burden, and the decision between a retrograde and a percutaneous approach becomes genuinely balanced. Staghorn Stones: The Largest Category
A staghorn stone is not simply a big stone. It is a stone that has grown to fill the renal pelvis and extend into the calyces, taking on a branched shape resembling a deer's antlers.
Most staghorn stones are struvite stones, driven by specific urea splitting bacteria rather than by diet or dehydration. That is why they grow so fast. A struvite stone can go from small to staghorn within months, while a calcium stone typically takes years.
Two consequences follow. First, treating a staghorn stone means clearing every fragment, because a single leftover piece harbours bacteria and the stone regrows. How Stones Get That Large Without Being Noticed
Patients are often shocked that a 4cm stone produced no severe pain. The explanation is straightforward.
Pain in stone disease comes from obstruction, not from the stone itself. A stone that stays inside the kidney and does not block urine flow can grow quietly for years. It is the small stone travelling down a narrow ureter that causes the agony, which is why a 4mm stone can be far more painful than a 30mm one.
Large stones therefore tend to announce themselves differently, through recurrent urinary infections, a persistent dull ache in the flank, blood in the urine, or an abnormality found incidentally on a scan done for another reason. Why Large Stones Are Dangerous Even Without Pain
A silent stone is not a harmless stone.

How Fast Do Stones Grow?
Growth rate varies enormously by type.
Calcium stones, the most common, generally grow slowly, often over years. Uric acid stones can grow faster when urine is persistently acidic. Struvite infection stones are the fastest by a wide margin and can double in a matter of months.

Stopping a Stone From Getting Bigger
A stone that is already formed will not shrink with diet, with one exception. Uric acid stones can genuinely be dissolved with medication that raises urine pH. Calcium stones cannot be dissolved by anything you eat or drink, which makes most of the home remedies circulating online useless for them.
What you can influence is whether the stone continues growing and whether new ones form. That means adequate fluid intake, reduced salt, moderate animal protein, normal dietary calcium taken with meals, and, for many patients, added citrate.
